OVERVIEW
The Seizure Response Checklist provides a structured record for responding to a suspected seizure or seizure-like episode during pet care. It is an attachment to SOP 2.13: Seizure Response Procedure and is designed to be used with the related incident, injury-response, and emergency veterinary transport procedures.
The checklist guides staff through immediate actions such as stopping the service, moving equipment away, protecting the pet from hazards, keeping hands away from the mouth, reducing noise and activity, and observing the episode. It also records approximate start and end times, owner contact, veterinary care recommendations, grooming decisions, and incident reporting.
A dedicated observations section provides space to document what happened before, during, and after the episode without replacing factual observation with a medical diagnosis.

Use this checklist with SOP 2.13: Seizure Response Procedure, SOP 2.06: Incident Reporting Procedure, SOP 2.07: Injury Response and Client Notification Procedure, and SOP 2.16: Emergency Veterinary Transport Procedure. Complete the response items as applicable, record the episode timing and observations, contact the owner immediately, and document veterinary recommendations when appropriate. Complete the Incident Report Form when required and record any future appointment requirements in the designated section.
