OVERVIEW
The Collapse or Fainting Response Checklist provides a structured record for responding to a pet collapse, fainting episode, weakness, or loss of consciousness. It is an attachment to SOP 2.15: Collapse or Fainting Response Procedure and is designed for use with the related emergency, injury-response, heat stress, seizure, and respiratory distress procedures.
The checklist documents immediate actions including stopping the service, turning off equipment, moving tools away, protecting the pet from injury, adjusting restraints when appropriate, relocating the pet to a quiet area when safe, and monitoring breathing and responsiveness. It also records owner communication, veterinary care recommendations, grooming decisions, and incident reporting.
The observation section provides space to document what occurred, the service being performed, equipment or exposure involved, loss of consciousness, recovery, owner response, and future appointment requirements.
